JOB SUMMARY-
The Home and Community Based Service Provider can provide Habilitation, Respite, and/or Attendant care and will provide services in order for an individual with a developmental disability to remain in her/her home and/or participate in work/community activities. The Home and Community Based Service Provider will assist the consumer to attain or maintain safe and sanitary living conditions and/or maintain personal cleanliness and activities of daily living. The Home and Community Based Service Provider will deliver services with compassion and respect.
ESSENTIAL FUNCTIONS-
1. Implement the general plan of care based on the ISP-approved Services Assessment or plan of care.
2. Enable the member to acquire the skills to be a valued member their community.
3. Provide assistance to maintain personal cleanliness and in activities of daily living that do not require medical supervision or intervention.
4. Provide training to increase the member’s self-help skills
5. Provide assistance by planning, shopping, storing, and cooking food for nutritional meals.
6. Ensure that the consumer receives medications as prescribed.
7. Provide appropriate attention to injury and illness.
8. Provide assistance to attain or maintain safe and sanitary living conditions.
9. Provide observation of consumer’s behavior, immediately reporting any potentially dangerous situations to the responsible person and supervisor.
10. Maintain accurate documentation of all required paperwork and submit timely.
PHYSICAL/MENTAL DEMANDS, ENVIRONMENT-
1. Schedule visits to coincide with family preference and routine.
2. Ability to work weekends and evenings as required by the needs of each member
3. Maintain professionalism at all times
4. Provide safe transportation to and from the home and community activities.
5. Possess organizational skills and the ability to multi-task.
6. Must have outstanding communication and social skills.
7. The ability to initiate tasks necessary for the program to run efficiently.
8. The ability to work well under pressure.
9. Ability to problem solve.
MINIMUM QUALIFICATIONS-
To function effectively in this position, the provider must:
• Possess a High School Diploma or G.E.D.
• Have experience working with the developmentally disabled population.
• Provider must have knowledge of the company policies and procedures.
• The Home and Community Based Service Provider must have a valid Arizona Driver’s License and clean driving record, and must have a reliable means of transportation with current insurance and registration
• A current Arizona Level One Fingerprint Clearance Card
• Completed criminal history affidavit
• Approved notice from the Department of Economic Security central registry.
JOB POSTING-
The Home and Community Based Service Provider must complete the following trainings:
• Article IX Training
• Adult/Child/Infant CPR, and First Aid
• The Home and Community Based Service Provider must complete required DCW training within 90 days of the hire date, if providing attendant care.
